Abstract:
The integration of community-based long-term care service resources is helpful to solving the problems of service shortage and fragmentation. Based on Valentijn's resource integration theory, this paper studies the path and dilemma of resource integration of community embedded long-term care model from macro, meso, micro and cross level dimensions by taking Shanghai as an example. The research shows that the realization of comprehensive and full integration of long-term care service needs four main prerequisites: real cooperation mechanism among multiple providers, dynamic and efficient case management, comprehensive environmental support and common long-term expectations. Therefore, we should promote the comprehensive and full integration of community-based long-term care service resources from seven aspects, which include constructing the financing mechanism, unifying the management organization, enhancing the community trust, and increasing the industrial support.
